Requirements

Develop your data review project report. Use the Data Review Project Report Template, linked in the Resources of this unit. This template is different from the Data Review Project Proposal Template used in Assessment 2. Authoritative sources should be integrated into the Evidence-Based Recommendations and Conclusion sections of the report.Data Review Project Report and Presentation.

Provide a minimum of three visual data summaries (for example, pie chart, graph, spreadsheet, and process map), two evidence-based recommendations from the literature, and one new insight. Executive Summary
The aim of the project was to investigate the rate of hospital readmissions for Advent Health for heart failure and coronary artery bypass in order to implement interventions that can improve the quality of care and in turn reduce the readmission rate for these patients in the organization. The 30-day readmission rate rule was used as a measure of hospital quality while patient safety was assessed by examining the mortality rates & the rate of complications for heart failure and coronary artery bypass patients.
Cardiovascular risk factors such as hypertension & smoking associated were associated with high readmission rates as well as non-adherence to the treatment regimen. The most common reason for readmission inpatient heart conditions included heart failure and ischemic heart disease while the most common complication after coronary artery bypass was post-operative infections while atrial fibrillation and pleural effusion prevalent reasons for readmission after coronary artery bypass.
Recommended interventions to reduce the readmission rates include comprehensive patient education; cardiac rehabilitation program for patients undergoing coronary artery bypass; comprehensive discharge planning and outpatient support; and effective follow-up care post-discharge.Data Review Project Report and Presentation.

Introduction
Coronary artery bypass surgery is a very costly procedure that is associated with high short-term readmission rates (Andres et al, 2015). Hospital readmissions rates for heart failure and coronary artery bypass were relatively high at for AdventHealth. According to the data, readmission rates for heart failure were 24.8% compared to the benchmark of 22% and Coronary Artery Bypass was 17.7% compared to the benchmark of 14.9% set by national according to the Hospital Care Data (AdventHealth, 2019). This paper will, therefore, provide an assessment report for the high readmission rates at the AdventHealth and make recommendations regarding interventions that can reduce the high readmission rates.
Literature Review
Rehospitalizations account for a large amount of healthcare spending. According to Zywot et al (2018) readmission rates for coronary artery bypass is an important indicator of the quality of care in a hospital because coronary artery bypass is the most regular performed cardiac surgery. This is supported by Dunlay et al (2014) who explain that early readmission is deemed as a metric of quality of care. According to Lin et al (2017), the key to lowering readmission rates in hospitals dependent on the provision of high-quality care within an inpatient setting. In addition, readmission rates can also be reduced by improving care transition after discharge.
Evidence shows that identifying patients at risk of readmission after coronary heart surgery is important in lowering the rate of readmission and in improving the quality of care in a hospital (Zywot et al, 2018). Therefore, improving the quality of care in a hospital is a major step towards reducing readmission rates.Data Review Project Report and Presentation. Factors attributable to high readmission rates in hospitals include poor discharge education, follow-up after discharge, communication gaps during discharge and poor quality of care during hospitalization (Iribarne et al, 2014). Poor quality of care during hospitalization can be contributed by factors such as increased workload, inadequate nursing staff, and medical systemic failures within the hospital (Lin et al, 2017). A randomized controlled trial conducted by Dunlay et al (2014) indicated that pre-operative physical exercise training, patient education, nurse-follow-ups, as well as cardiac rehabilitation after coronary artery bypass reduced the overall length of hospital stay, as well as the readmission rates. According to Osnabrugge, et al (2014)readmissions over a short period may indicate the quality of care administered in a healthcare organization while longer timeframes such as 90 days may reflect the quality of community care. Therefore, assessing the readmission rates with as an indicator of the quality of care should focus on readmission rates over a short period of time such as seven days.Data Review Project Report and Presentation.
Data Review
Description ofdatameasuredfor theproject
• The 30-day readmission rate rule was used as a measure of hospital quality. Logistic regression was used to predict the likelihood of readmission for each patient with heart failure and patients who had undergone coronary artery bypass. Projected readmission probabilities at the hospital were then summed up in order to estimate the expected number of readmissions. The ratio of observed to projected readmissions was then calculated. The figure was then multiplied by the standard readmission rate in order to identify the risk-adjusted readmission rates at the hospital

The above figure indicates a relatively high readmission rate for patients undergoing coronary bypass surgery at the hospital after months. From the above graph, the readmission was high and keeps increasing with the progressing months.Data Review Project Report and Presentation.

Figure 2 indicates that the readmission rate for patients undergoing coronary bypass at the hospital was highest within the first 30 days after discharge.
Data Analysis
Analysis of the hospital data indicated that cardiovascular risk factors such as hypertension & smoking associated with high readmission rates as well as non-adherence to treatment regimen attributable to high readmission rates. Patients staying long in ICU and patients with longer hospital length of stay have higher readmission rates. Majority of readmissions occur early post-discharge. The most common reason for readmission in patient heart conditions included heart failure and ischemic heart disease. The common complication after coronary artery bypass was post-operative infections while atrial fibrillation and pleural effusion prevalent reasons for readmission after coronary artery bypass
Rationale for Conducting the Project
The hospital readmissions for AdventHealth rate for patients with heart failure is relatively high when compared to the benchmark set by national according to the Hospital Care Data (AdventHealth, 2019). Therefore, the aim of this project was to investigate the rate of hospital readmissions for Advent Health for heart failure and Coronary Artery Bypass in order to implement interventions that can reduce the readmission rate for this patient population.Data Review Project Report and Presentation.
Recommendationsfor theFuture
• Heart failure & coronary artery bypass patients should receive comprehensive patient education. This is because adequate patient education ensures adherence to the treatment regimen, helps in self-monitoring and informs patients on risk factors to avoid (Jill et al, 2015).
• The hospital should consider providing a cardiac rehabilitation program for patients undergoing coronary artery bypass. This is because cardiac rehabilitation has been shown to significantly reduce readmission rate for patients with heart failure and have undergone treatment (Andres et al, 2015)
• Comprehensive discharge planning along with outpatient support can lower the readmission rates and also improve patient outcomes for patients with heart failure and coronary artery bypass patients. This should include integrating ambulatory care and multidisciplinary teams in discharge planning and care transition (Ziaeian& Gregg, 2016).
• Effective follow-up care. This should include nurses making follow-up telephone calls and community nurses making home visits to monitor the patients’ progress and adherence to treatment (Jill et al, 2015).
Conclusion
The readmission rate for heart failure & coronary artery bypass patients high at Advent Health and therefore high-quality care within an inpatient setting can reduce readmission rates in the organization. Factors that contribute to high readmission rates for this set of patients include post-operative infections, atrial fibrillation, and pleural effusion, non-adherence to treatment regimen & risk factors to heart disease. Therefore, improving care transition after discharge can significantly reduce the readmission rates. Interventions that can be used to reduce readmission rate for heart failure and coronary artery bypass include comprehensive patient education and discharge planning, cardiac rehabilitation programs and effective follow-up care after discharge.Data Review Project Report and Presentation.
