Roger’s diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck’s change curve model or the transtheoretical model of behavioral change. Other conceptual models, such as a utilization model (Stetler’s model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based proposal in clinical practice. Roger’s Diffusion of Innovation Theory and its Relevance
According to LaMorte (2018), the diffusion of innovation theory serves to explain how an idea, practice, or innovation gains ground and spreads or diffuses throughout the organization from the time of its introduction. The theory was put forward by Roger in the early 1960s and its main outcome is the adoption of a new practice (in this context), hitherto not exercised in the organization. The successful diffusion of the new practice (in this case prescribing exercise for hypertension) means a change in practice from the previous way of doing things. It is the healthcare professionals that have to adopt and practice this new evidence-based modifiable lifestyle treatment modality. As such, for successful implementation and adoption the adopters have to see the practice as new, innovative, adding value, and beneficial to their patients (LaMorte, 2018). This is because inevitably some will adopt the practice earlier than others, since adopter characteristics fall into different categories or “degrees of innovativeness” (LaMorte, 2018; Dearing & Cox, 2018). The above explanation shows clearly how this theory is relevant to the implementation of this project of change in practice of hypertension management. We have already looked at the organizational culture, and now appreciate that there are going to be health workers ranging from innovators to laggards.
Stages in Roger’s Diffusion of Innovation Theoretical Framework
The stages of adoption or the process of the decision to adopt include (i) awareness or knowledge (here the potential adopters have to be given the knowledge of the existence and efficacy of the intended new practice), (ii) decision to adopt/ reject or persuasion (peers, colleagues, and management convince the staff of the importance of adopting the new practice), (iii) initial trial of the practice (the healthcare workers try out the new idea to see if it indeed is worthwhile), (iv) confirmation of practice of continuation of use (the idea/ practice gets entrenched in the organizational culture and becomes routine with sustained use) (Pashaeypoor et al., 2016; LaMorte, 2018).Evidence-Based Practice Proposal – Section D: Change Model.
Applying Above Stages in the Proposed Exercise-in-hypertension EBP Implementation
Exceptional patient care relies on healthcare workers who use EBP (Pashaeypoor et al., 2016). In awareness, the staff will be taken through several hours of continuous medical education (CME) on the subject of exercise in hypertension treatment. In persuasion, the departmental heads will be tasked with motivating their staff to adopt the change and show results. In initial trial there will be positive reinforcement by way of public acknowledgement of the early implementers by the management.
