Translation Science Project Assignment Discussion Paper

Evidence-based practice is one of the most common subjects, particularly in the medical field, and it is important in keeping up with the changing market dynamics in healthcare. Whenever individuals seek medical treatment at different health institutions, they anticipate receiving care from medical experts who are skilled and experienced. Evidence-based practice is one of the ways that allows health workers to be competent in guaranteeing adequate patient care while at the same time-saving time. As a result, people who seek healthcare services will be more satisfied if this technique is implemented. Diseases like major depressive disorder are a source of distress for individuals of all ages. Individuals suffering from the major depressive disorder may find it difficult to carry out their tasks as anticipated, and their overall quality of life may suffer as a result. As a result, the goal of this study is to present major depressive disorder as a practice issue, as well as an evidence-based solution to handle it. A literature synthesis connected to the practice issue will be supported by the content, which will help in developing professional formulation, communication, and dissemination abilities that will be useful as a DNP student Translation Science Project Assignment Discussion Paper.

Practice Problem and Question

Major depressive disorder (MDD) is a mental condition that is quite common and may be recurring or even chronic at times. Approximately 6.7 percent of the population aged 18 and older in the United States suffers from MDD in any given year, according to the Centers for Disease Control and Prevention. The World Health Organization has already gone so far as to estimate that depression would be the second most common cause of disability or early mortality by the year 2020. Among US individuals, the economic burden of major depressive disorder has grown significantly, rising from $US236 billion in 2010 to $US326 billion in 2018 (Pérez-Sola et al., 2021). Because of improved employment prospects for persons with MDD, the percentage attributed to workplace expenditures grew from 48 to 61 percent.

In the last several years, many healthcare workers, particularly nurses, have faced difficulties in diagnosing a major depressive disorder and ensuring that adults recover from this mental health condition. According to Drageset et al. (2016), depression may cause emotions of despair, and individuals who suffer from it may lose interest in a multitude of activities. Certain symptoms, such as sleeplessness, exhaustion, headache, and anxiety, may have a negative impact on the patient’s overall quality of life. Patients may have suicidal thoughts or turn to drug and substance dependence if they are not diagnosed and treated promptly (Van der Zweerde et al., 2019)Translation Science Project Assignment Discussion Paper. Patients suffering from the major depressive disorder may also experience worsening of underlying health conditions. As a result, early diagnosis and treatment may improve the emotional well-being of adults, strengthen familial ties, and minimize the need for depression therapy in the healthcare system.

The Patient Health Questionnaire-9 (PHQ-9) is the solution that is proposed. In 2011, the Health Maintenance Organization started using this instrument in primary care settings (Patel et al., 2019). The instrument is a self-report questionnaire that may be used to measure the severity of depressive symptoms as well as the occurrence of these symptoms. It has been found to be beneficial in the diagnosis of major depressive disorder.

The PICOT question, in this case, will be: In adults aged 20-65, does use of Patient Health Questionnaire-9 during regular family practice visits, compared to no use of Patient Health Questionnaire-9, significantly improve the detection and prompt treatment of major depressive disorder?

Evidence Synthesis of Literature

In searching for evidence in this practice issue, two databases were used, including Google Scholar and Cochrane Library. The keywords include major depressive disorder, depression, adults, diagnosis, patient health questionnaire-9, and phq-9. The search yielded multiple articles, which were further narrowed by utilizing the Boolean Operators AND and OR to narrow the search results. This resulted in the discovery of articles that included pertinent information concerning the use of phq-9 in the detection of depression in adults.

The literature review offered an answer to the practice question and served as a roadmap to achieving the goal of this project. Depression screening is recommended by Levis et al. (2019). According to the clinical team’s statistics, only 50 percent of the patients were assessed using the PHQ-9 instrument, which is lower than the national average of 60%. The data provided above for the clinic falls short of the minimum criterion of 100 percent for all practitioners (Levis et al., 2019). Molebatsi et al. (2020) reinforced the hypothesis that the greater the number of clinicians who evaluate and treat depression using the PHQ-9, the greater the likelihood that patients would eventually have lower depression screening scores, resulting in better patient outcomes and a better quality of life. It was also pointed out that the use of PHQ-9 screening instruments at a Sleep Study Office is critical in the diagnosis and management of depression. According to Sun et al. (2019), evidence suggests that depression screening is critical in ensuring early detection and improved treatment outcomes. Ample evidence was also discovered to support the claim that screening for depression is not harmful. Sun et al. (2019) also stated that practitioners should utilize the PHQ-9 assessment on a regular basis to detect signs of depression early, improve early treatment efforts, and reduce the risk of severe complications. According to the National Institute of Mental Health (2017), most successful suicides were committed by individuals who sought medical attention in the week preceding their death. This DNP project had a number of important objectives, one of which was to guarantee that the clinicians received evidence-based education in order to use the PHQ-9 instrument more regularly and to increase their level of confidence in utilizing it. According to the findings of literature analysis and evaluation, Sun et al. (2019) concluded that a lack of education had a detrimental influence on the use of PHQ-9 screening instruments to diagnose and treat depression. The Patient Health Questionnaire-9 (PHQ-9) has been found as a viable and accurate instrument for screening for depression in a Sleep Study Office (Smith et al, 2019). According to a number of independent research studies, employing the PHQ-9 depression-screening instrument after training healthcare practitioners helps to improve early diagnosis and fast treatment of depression (Smith et al, 2019)Translation Science Project Assignment Discussion Paper.  Depression is a contributory element to rising impairment across the world, according to Williams, Chung, and Muennig (2017), and they cost the United States an estimated 233 billion dollars in 2016. Depression has a negative impact on the economy since it leads to greater healthcare utilization, worse quality of life, and reduced job performance (Williams et al., 2017). Over 16 million individuals in the United States had at least one major depressive incident in 2016 (Williams et al., 2017). Failure to detect depression may result in serious harm for both patients and communities. Patients should be adequately screened, diagnosed, and treated by clinicians in the hospital if they are suffering from depression. Although depression is common in the United States and clear instructions for using the PHQ-9 exist, physicians tend to face challenges in incorporating this depression screening tool into their daily practices (DeCapua, 2018). After reviewing, comparing, and analyzing the literature, I came to the conclusion that the use of PHQ-9 screening tools by clinicians is critical in the diagnosis and management of depression. Today’s healthcare system demands that healthcare practitioners be educated on the use of PHQ-9 since it has the potential to enhance patient treatment results and general care quality (DeCapua, 2018). As a result, clinicians must obtain enough training on the PHQ-9 in order to effectively treat patients’ depression. Based on the findings of the literature review, a training program is designed to improve the expertise, confidence, and clinical outcomes of healthcare workers in a cost-effective and favorable way.

Data-Driven Decision-Making

An approximated 21.0 million individuals in the United States have had at least one major depressive episode (Kramarow & Broddy, 2020). A variety of probable factors, including a rise in the incidence of depressive disorders, have been suggested as causes for this trend. Since 2010, there has been a significant increase in the incidence of major depression among adults in the United States. Since the beginning of the twenty-first century, research has shown a considerable, albeit less noticeable, rise in internalizing difficulties and depressive symptoms among adults. There are a variety of possible explanations for this rise, including an increase in bullying and victimization, as well as increased usage of social media and technology, which may have been more detrimental to adults (Kramarow & Broddy, 2020). In addition, decreased sleep quantity and quality, the long-term effects of the Great Recession, and growing educational expectations may all have had a role in the increase in major depressive disorder (Kramarow & Broddy, 2020).

With the help of information technology (IT) systems, patient engagement, quality of care, safety and cost-effectiveness are all being enhanced (Timmins et al., 2018). HIT has benefited healthcare by streamlining care coordination, providing care more efficiently, lowering mistake rates, and increasing medical outcomes. The utilization of technology in the healthcare delivery process has both good and bad elements, and both must be considered. Healthcare professionals have a responsibility to safeguard their patients, even if the risks outweigh the benefits. Technological advancements and innovations have the ability to aid in the fight against the major depressive disorder. Apps and devices that make it easy to track changes in mood are just a few of the cutting-edge depression solutions that are now available (Timmins et al., 2018). As a result, healthcare institutions and providers must constantly evaluate how their operations impact patient care Translation Science Project Assignment Discussion Paper.

Conclusion

One of the disorders that nurses should be able to treat effectively with the use of evidence-based techniques is major depressive disorder (MDD). This illness is widespread and continues to cause people to have severe symptoms and problems. adults, for example, may become disinterested in their activities and develop suicidal ideas. As a result, addressing this illness is essential for improving the health of adults. A correct diagnosis is required before treatment can be started. Depression, on the other hand, is a difficult condition to diagnose. According to research, the use of the patient health questionnaire-9 may be utilized to detect depression in its early stages.
